Thieme TeleCare is a German healthcare provider specializing in digital coaching programs for patients with chronic conditions.
The project team consisted of Wolfgang Weber (Managing Director), Lukas Daub (Project Manager), Daniel Lorenz (Market & Sales) and Carina Ziegele (Marketing). Each of them brought unique expertise to the collaboration, from market insights to technical implementation.
Thieme TeleCare uses Patient Journey App to provide a digital tool that healthcare insurance companies can provide to their patients. They have over 100 treatments live and continue to improve and add new treatments in the coming year.

Why Patient Journey App
The direct clients of Thieme Telecare, healthcare insurances, want to become more digital and Patient Journey App enables them to do just that.
“Patient Journey App gives us the possibility to build smart pathways with dynamic conditions and monitoring. With Patient Journey App, we can adapt programs quickly, learn from our peer group, and improve continuously.
For example, a patient gave feedback that certain information about the peak flow was not very clear. We discussed this feedback with our medical advisor and were able to change it ourselves in a few hours and now the information is up to date again”.

Benefits of Patient Journey App
For Thieme TeleCare it was very important to have an app with a clear and easy to understand interface. There is a clear path that shows every step of their journey and an information area where they can always find everything available to read about their illness.
Due to the large variety of treatments offered in the app, patients of all ages are using it daily. Even though younger people may be more accustomed to using digital solutions in their daily life, there are patients of over 70 or 80 years old who use them regularly. Thieme TeleCare saw a real shift in the last couple of years in the use of apps by the older population.
Additionally, a benefit for patients is that the app provides videos and text that can be watched and read over and over again. Healthcare professionals do not have the time to repeat information multiple times.
Lastly, Patient Journey App can be used for prevention. The app can help prevent patients from getting sick or sicker.

Feedback from patients
Collecting feedback is a very important part to continuously improve the app further.
Patients are asked to fill in satisfaction questionnaires through the app and are contacted by phone by health managers.
In addition, the data provided by the dashboard in Patient Journey App is used.
Lucas explains: “for example, we see an exercise is never used and the textbox to provide feedback is always empty, then we can change the exercise and monitor the effect”.
The overall feedback from patients is that they receive the app as very valuable and are using it in their daily life.
Georg - Virtual health guide
Some illnesses, like for example high blood pressure have a huge physical and emotional impact on a patients’ daily life. Georg makes the topic a bit lighter and supports patients as a personal health guide.
“People don’t expect Georg when they open the app, he is a friendly welcome at the start of their journey.”
